Intake Forms

Prior to our visit, you will receive an email with a link to my intake forms through MilkNotes. Please fill them out in detail along with signing the “consent for care”. (MilkNotes is a secure HIPAA compliant medical charting platform)

It is important that I understand both you and your baby’s health history to better help you navigate challenges. We can schedule a 15-30 minute virtual visit prior to our in-person visit to go over your history and main concerns. This will expedite our visit and allow us to spend our time focused on you and your baby. Thank you!

Home Visits

Preparing yourself, your little one, and your space:

Trying to feed your baby 1.5 to 2.5 hours before I arrive is best - a hungry, but not starving baby at the beginning of the visit provides time for us to discuss your intake forms, perform any breast/chest exams, and to assess your baby. With that said, infants are unpredictable, so if your little one is starving, consider giving a snack before I arrive, it will not be the end of the world. 

Have your baby in a fresh diaper with no onesie on so that we can weigh your baby before feeding. We will do a before and after weight to determine how much milk your baby is transferring in that session. If you are using any breastfeeding support items, such as, breast pumps, bottles, nipple shields, etc., have those accessible. Please put pets in another room to eliminate distractions.

Please DO NOT spend time cleaning your house in preparation for our visit. Your family is in postpartum life; your only responsibility right now is caring for your new baby. Rest assured that my attention will be on YOU and your baby and not on the dust bunnies or dirty dishes! You are welcome to invite any support people to our visit. The more support the better as long as you are comfortable with them there.

Office Visits

My office in Holladay is a private, safe space with everything that I bring to a home visit. Be sure to bring any breastfeeding support items that you are using such as, breast pumps, bottles, nipple shields, burp cloths, etc.

Virtual Consults

Please make sure you have a solid internet connection prior to our virtual consult. Please complete your intake forms 24 hours prior to our visit. You will find a tab that says VideoCare on the top bar of your MilkNotes home page. Simply click that tab and go through the steps to ensure your camera and sound are working. Have your baby ready and any other breastfeeding props on hand that you are using. Please have pets or older children in another room or with a caregiver to eliminate distractions. Cancellation Policy

I have a 24-hour cancellation policy and a $50 fee for cancellations after 24 hours. I understand life can get complicated so let me know as soon as possible if you have a conflict and we can reschedule for a better time.
